Danger – Ultraviolet Radiation WEAR PROTECTIVE EYEWEAR; FAILURE TO MAY RESULT IN SEVERE BURNS OR LONG TERM INJURY TO THE EYES. As with natural sunlight, overexposure can cause eye and skin injury and allergic reactions. Repeated exposure may cause premature aging of the skin and skin cancer. Consult physician before using sunlamp if you are using medications or have a history of skin problems or believe yourself especially sensitive to sunlight. If you do not tan in the sun, you are unlikely to tan from the use of this product. Children, the elderly, or fair skinned people who always burn easily and either never tan or tan minimally should not use this equipment. Never tan more than once a day. Tan normally appears after the first few sessions and maximizes after the first four weeks. Consumers should report to the agency any injury for which medical attention was sought or obtained resulting from the use of registered tanning equipment. This report should be made within five working days. I, recognize that there could be dangers inherent in tanning for some individuals I acknowledge that the possibility of certain unusual physical changes during tanning does exist. I have been advised of the following risk in connection with my use of the tanning bed at The Harrell Organization, LLC dba Tropical Tan Studio and understand I am tanning at my own risk. A person who uses a tanning device in this tanning facility must use eye protection. If the provided eye protection in not worn, use of tanning device in this facility may cause damage to the eyes. Overexposure to the ultraviolet radiation produced by a tanning device in this facility may cause burns. Repeated exposure to the ultraviolet radiation produced by the tanning devices in this tanning facility may cause premature aging of the skin, skin thickening and skin cancer. Abnormal skin sensitivity to ultraviolet radiation or burning may be caused by certain foods, cosmetics, or medication, including but not limited to the following; Tranquilizers , Diuretics, Antibiotics, High blood pressure medication, Birth control medication, other photosensitizing agents An Individual who is taking prescription drug or over-the -counter drug should consult a physician before using a tanning device. Acknowledgement and Agreement I acknowledge and agree that I assume the risks associated with this service. I acknowledge and agree that now warranties or representations have been made to me regarding the results I will achieve. I acknowledge that The Harrell Organization, LLC dba Tropical Tan Studio is not promoting medical or health benefits from use of our tanning beds. I understand that the results are individual and may vary. I understand that I will not be allowed to exceed the maximum allowable time posted on the tanning device. I have been given adequate instructions for the proper use of the tanning equipment. Prior to my initial exposure, I was given the opportunity to read the warning and consumer statement above. I have read, understood and completed this questionnaire and release of liability. Any questions I had were answered to my full satisfaction.
